Application form templates

An application form decides who gets something: a job, a home, a stall at the market, a scholarship. That is what separates it from a registration form, which signs up anyone who asks. Each template here collects what a decision-maker needs to compare people fairly, asks for sensitive details only at the point they are needed, and leaves out the questions that invite a discrimination complaint. Every one is a working form you can publish today, with a printable PDF of the same questions.

Questions about application forms

What is the difference between an application form and a registration form?

An application asks to be chosen; a registration signs up anyone who qualifies. If you will review the answers and say yes or no, start from an application template. If everyone who fills it in gets a place until you are full, use a registration template.

Can applicants attach a CV, résumé or supporting documents?

Yes. Add a File upload question and choose which file types it accepts. Each file can be up to 10 MB on the free plan.

Can I stop taking applications on a closing date?

Yes. Set a close date in Settings and the form shows your closed message from then on. You can also cap the number of applications with a response limit.

Should the first form ask for a Social Security or national insurance number?

Rarely. These templates leave government ID numbers out of the first stage: you only need one once you have decided to go ahead, and collecting it from everyone means storing sensitive data you will never use.
