What’s inside the volunteer application form template

This volunteer application form collects everything you need to place someone in a role: contact details, the roles and shifts they want, when they can start, their experience, the checks they already hold, two referees and an emergency contact. Run the same volunteer form for every intake round by adding the program name to the link.

Why this template works

  • Applicants can upload a Working with Children Check or National Police Check as a PDF or a photo, and the upload only appears when they say they hold one.
  • Under-18 applicants are asked for a parent or guardian, and finish on a screen that says a consent form is on its way to them.
  • A required consent box covers asking for a police check, and the last screen sets out the chat, the referee call and the induction.

Questions about this template

Is this volunteer form free?

Yes. Unlimited responses, file uploads up to 10 MB, logic and two thank-you screens are all on the Free plan, under the fair-use policy. You do not need an account to build it, only an email address to publish it.

Can volunteers attach a police check or a Working with Children Check?

Yes. The form asks which checks they hold, then shows an upload question for a PDF or a photo of the card, up to 10 MB per file. It also asks for the Working with Children Check number and expiry. Anyone who holds none of them sees a line saying you will tell them which check their role needs.

How do I ask different questions for volunteers under 18?

The form already does. A yes or no question asks whether the applicant is under 18. If they are, the parent or guardian name and email appear and are required, and that application ends on a separate screen about the consent form.

Can I use one form for several intake rounds?

Yes. Add ?program=Summer festival&round=2026-summer to the link you share. The heading names the program and every response is tagged with the round, so you can sort last summer's applicants from this one's.

Who can see the applications?

Only you and anyone you share your PulseFormCreator account with. Responses are saved in your dashboard and export to CSV or Excel. Turn on "Email me" to get each one in your inbox, or send them to Google Sheets, Airtable or Slack.

Does it handle rosters and shifts after someone is accepted?

No. It records the shifts each person says they are free for, which is what you need to place them. Rostering itself happens in your spreadsheet or roster tool.
