About PulseFormCreator
PulseFormCreator is an independent online form builder. It builds application, order, registration, consent and contact forms, with logic, a signature field and file uploads on the free plan. Michael Hodge founded it as Pulseware in 2006 and still runs it. My Plaza Pty Ltd operates it.
We built PulseFormCreator because we run forms and surveys ourselves, at the start of projects and after launches, and wanted an editor we could open and start typing in. You can build a whole form without an account. When you want its link, you save it with your email address and a one-time code, and the form goes live straight away.
The editor has 24 question types and 8 layout blocks, conditional logic, calculations and scoring, 8 embed styles, AI drafting, and CSV and Excel export. All of that is on the Free plan. The features page lists every feature with the plan it needs.
We do not train AI models on your form content and we do not sell respondent data. The privacy policy, the security page and the AI data policy say what we store and who processes it.

Michael founded Pulseware in 2006 and still runs it as PulseFormCreator. He directs form design and content quality: how each question is worded, which answer options it offers and how the answers can be scored. He is the author of every template in the template library. His background is questionnaire methodology and item construction, from formal psychology training at the University of Wollongong (Bachelor of Science, Psychology) and twenty years of building online surveys and assessments.
Michael also runs SeeMyPersonality, free personality tests built on published instruments with transparent scoring, and myiqtested.com, a free cognitive assessment built on the open-source ICAR framework.
Why PulseFormCreator is free
The Free plan has unlimited forms, questions and responses under a published fair use policy. The Pro and Business plans are how PulseFormCreator earns money. They add things a business pays for: removing the PulseFormCreator badge, confirmation emails to respondents, drop-off insights, email verification and data retention rules. Response volume is never the reason to upgrade. Pricing has the detail.

How we run the product

Paid features are labeled
Every feature is listed on the features page with the plan it needs. The editor marks a paid setting before you turn it on.

Your data is exportable
CSV and Excel export is on the Free plan. You can delete a response, a form or your account at any time.

Forms are built for phones
The form people fill in is designed for a small screen first. On a phone, a grid question turns each row into its own card, so nothing scrolls sideways.
