About PulseFormCreator
PulseFormCreator is an independent online form builder used by teams around the world. One editor builds contact, registration and order forms, surveys and quizzes. It is operated by My Plaza Pty Ltd and run by its founder, Michael Hodge.
We built PulseFormCreator because we run forms and surveys ourselves, at the start of projects and after launches, and wanted an editor we could open and start typing in. You can build a whole form without an account. We ask for an email address when you publish, because that is where the responses are sent.
The editor has 22 question types and 8 layout blocks, conditional logic, calculations and scoring, 8 embed styles, AI drafting, and CSV and Excel export. All of that is on the Free plan. The features page lists every feature with the plan it needs.
We do not train AI models on your form content and we do not sell respondent data. The privacy policy, the security page and the AI data policy say what we store and who processes it.

Michael directs assessment design and content quality at PulseFormCreator. The approach combines formal psychology training at the University of Wollongong with hands-on expertise in questionnaire methodology, item construction, and the practical psychometrics of online survey design.
Outside PulseFormCreator, Michael also runs myiqtested.com, a free cognitive assessment built on the open-source ICAR framework. He writes there about what a single test score can and cannot tell you.

Why PulseFormCreator is free
The Free plan has unlimited forms, questions and responses under a published fair use policy. The Pro and Business plans are how PulseFormCreator earns money. They add things a business pays for: removing the PulseFormCreator badge, confirmation emails to respondents, drop-off insights, email verification and data retention rules. Response volume is never the reason to upgrade. Pricing has the detail.
PulseFormCreator does not take payments inside a form, and it has no custom domains, team workspaces or public API. If you need those, another product will suit you better. We keep dated comparisons with Tally, Typeform, SurveyMonkey, Google Forms and Jotform, and each one lists where the other product is ahead.
How we run the product
- Paid features are labelled
Every feature is listed on the features page with the plan it needs. The editor marks a paid setting before you turn it on.
- Your data is exportable
CSV and Excel export is on the Free plan. You can delete a response, a form or your account at any time.
- Forms are built for phones
Most people answer on a phone, so the respondent view is designed for a small screen first.
