Request forms

A request form starts a small process: someone asks, and someone else approves it or does the work. These request form templates cover both kinds. Staff requests, such as time off, purchases and travel, carry what an approver needs to say yes. Customer requests, such as repairs, service calls, quotes and appointments, collect enough to book the job without a call.

Questions about request forms

Can a manager approve a request?

Yes, on Pro. Add an Approval step in the Workflows tab: each request waits for the approver’s OK, and they approve or reject it from their inbox, with no account needed.

Can I send different requests to different people?

Yes. Each notification can have its own recipients and an “Only when answers match” condition, free, so IT requests reach IT and repair requests reach facilities. Inspections work the same way: a vehicle inspection checklist emails whoever books repairs only when a driver marks a defect, and a roof inspection form emails your estimator only when a roof rates Poor.

Can people attach a receipt, quote or photo?

Yes. Add a File upload question and choose which file types it accepts. One question can take up to 20 files, each up to 10 MB on the free plan.

Can I reply to the person who asked?

Yes, when the form asks for their email. Open the response and send a reply, free: their answer comes back into the same thread.

Which form covers a sick day, or coming back after one?

A missed day is recorded afterwards on an absence report form, not asked for in advance. After a longer illness or an injury, a return-to-work form records the provider’s release before the person starts again.
