What we left out
- A notary block. Only Miami-Dade’s permit form has one. A permit office that needs it has its own form.

- Core samples and moisture tests. Lab work for a replacement design, not a condition report.

In Florida an insurer may not refuse or drop a home policy on roof age alone if an authorised inspector finds five or more years of life left. Florida Statutes 627.7011

Questions about this template
What does a roof inspection report look like?
Like the filled-in example above: the roof, a grade for each part, photos, remaining life and priced repairs.
Will an insurer accept it?
Florida’s Citizens accepts a form with at least the detail of its own. Pick the insurance preset, which follows its fields, and check with the insurer first.
What does it mean when a report says the roof is at end of life?
That the inspector expects the covering to need replacing soon: here, “Under 2 years”. The repairs box says what keeps it watertight until then.
Should the inspector fill it in on the roof?
No. Take photos on the roof and fill in the form at the ladder or in the truck.
