Inspection, report and evaluation form templates

These forms turn an observation into a record: the state of a vehicle before a trip, the condition of a flat on the day someone moves in, what happened in an incident, how well a course or a lesson went. A record is only useful if it is filled in the same way every time and can be found later, so each template here uses fixed ratings or checklists instead of free text wherever it can and stamps every submission with the date it was sent, and the inspection and incident forms take photos from a phone.

Questions about inspection, report & evaluation forms

Can inspectors take photos with their phone?

Yes. A File upload question opens the camera on a phone, so a photo of a defect goes straight into the report. Each upload question takes one file of up to 10 MB on the free plan, so several photos go in as one PDF.

Can I see every report in one table?

Yes. Every submission lands in your responses table with its date and time, and you can export all of them to CSV or Excel or send them to a Google Sheet as they arrive.

Can a report be filled in without internet?

No. The form needs a connection to submit, and photos upload as they are added. Typed answers stay in the browser until they are sent (Save respondents’ progress, in Settings → Responses, is on by default), so a lost signal or a closed tab loses nothing typed.
