What happens after someone submits

Set these up in the Workflows tab. AI triage runs only on forms where you turn it on.

  1. Submitted

  2. A student submits

AI triage adds a sentiment and tags, so comments about the same problem group together: 100 a month free.

  3. Each week

One digest lists the new evaluations and, on Pro with triage on, opens with an AI summary of what students said.

  4. The term ends

Export every answer to CSV or Excel, with the course and cohort from the link in their own columns.

What this form asks, and why

The last column shows how often real course evaluation forms ask for it.

Question Why it is there Real forms
Overall, how would you rate this course? Required The one number to track from term to term. The link names the course. 2 of 11 forms
How much do you agree about the course? Six statements, each about one thing, as Purdue rewrote its own. Any row may be skipped. 5 of 11 forms
Overall, how would you rate the teaching? Required Asked by 4 of 11 forms, more often than an overall course rating. 4 of 11 forms
How much do you agree about the teacher? Kept apart from the course, as UF does. 8 of 11 forms ask whether the teacher explained clearly. 8 of 11 forms
The pace was… Only 2 of 11 forms ask, but it explains a low rating faster than any comment. 2 of 11 forms
Compared with your other courses, the workload was… Against other courses, as IUP asks, because “heavy” means little on its own. 1 of 11 forms
Which topics needed more time? Only when “The pace was…” is “Too fast” or “Compared with your other courses, the workload was…” is “Heavier” or “Much heavier” Only for students who found it too fast or too heavy. Our own recommendation
Why did you take this course? A required course is rated differently from a chosen one. 4 of 11 forms ask. 4 of 11 forms
What helped you learn the most? On 7 of 11 forms. Western Washington now offers an evaluation of open questions only. 7 of 11 forms
What should change next time? Asks for the fix, not the complaint. 6 of 11 forms

What we left out

  • Expected grade. 3 of 11 forms ask. In a small class it helps identify who wrote what, which undoes the anonymity.
  • A 0–10 “would you recommend” score. Only 1 of 11 forms asks it. Many courses are compulsory, so “would you recommend it” has no clear answer.
  • A comment box under every statement. It doubles the length. Two open questions at the end catch the same points.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When “The pace was…” is “Too fast” or “Compared with your other courses, the workload was…” is “Heavier” or “Much heavier”

shows “Which topics needed more time?”

Make it yours

  1. One form for every class

Add ?course=Biology 101&instructor=Dr. Patel to each class’s link. The questions name the course and the teacher.

  2. Add Harvard’s optional questions

Harvard Extension’s form also rates the teaching assistant and asks how students attended (in person, live online or recorded), hours a week on the course and how hard it was. Use Add in the Questions tab.

  3. Close it before results

Settings → Access and limits → Schedule: close the form the day before grades are released.

  4. One answer per student

Turn on Prevent duplicates, once per browser, in Settings → Access and limits. It still works with anonymous responses.

Questions about this template

Are the answers really anonymous?

Yes. Anonymous responses are on, so no IP address, browser or device is stored with an answer. In a very small class, a comment can still give someone away.

When should students fill it in?

In the last week, before final grades. Pomona’s form tells students that instructors see no results until grades are submitted.

Can I compare one term with the next?

Yes. Add ?cohort=2026-1 to the link. The cohort is saved with every answer and gets its own column in the CSV or Excel export.

Can I use it for a training course at work?

Yes. Drop the rows that do not apply, such as grading. Purdue lets departments leave out its lab and exam questions for the same reason.

Which form do you need?

Not quite the job? One of these may fit, or browse all inspection, report and evaluation forms.
