What this form asks, and why

Question Why it is there
Overall, how would you rate the event? Required The number to compare one event with the next.
How was each part? Required Each part rated apart, so a cold room does not sink the speakers. “Didn’t use” keeps the lunch score honest and lets in-person guests skip the stream row.
Did it give you what you came for? Required What people came for is the real measure, and it differs from how much they enjoyed it.
Which part was most useful, and why? Tells you what to keep. Asking “why” turns praise into something you can repeat.
What should we change next time? Asks for a change, not a complaint.
What will you do differently after today? For training days and conferences: what people will actually do differently.
Would you come again? For a one-off event, the answer you can plan the next one around.
How did you hear about it? Shows which invitations worked, so next time the budget goes there.
What should a future event cover? Your next programme, suggested by the people who will attend it.
Quotes Unticked. Quotes go in your publicity only with permission, and without a name.

What we left out

  • Age, gender and ethnicity. Ask them only if you must report on who you reach. Otherwise they lengthen the form and cost trust.
  • A 0–10 recommend score. The old event feedback survey asked it. “Would you come again?” asks about the person answering, which they can actually know.
  • Name and email. Anonymous answers are more candid. The testimonial box asks permission instead.

Make it yours

  1. One form, every event

    Add ?event=Spring Volunteer Forum to the link and the first question names the event.

  2. On the last slide

    Share → QR code gives a code for the closing slide or the doors, so people answer before they leave.

  3. Close it after a week

    Settings → Access and limits → Schedule: close it seven days after the event, while answers are still about this one.

Questions about this template

Can attendees rate each speaker?

Yes. Add a row per speaker to the “How was each part?” grid, or pick the workshop preset, which rates the presenter.

Should event feedback be anonymous?

This form is: no IP address or device is stored with an answer. Turn off Anonymous responses in Settings → Privacy if you want to follow people up.

Where did the event feedback survey template go?

It became this form, and old links land here. The overall rating, the rating of each part and “what should we change” stay, and online viewing is a row of its own. The recommend score and email sign-up went, for the reasons above.

Can I use it for a workshop or training day?

Yes. Pick “Workshop or training” above the preview: the grid asks about objectives, relevance and the presenter instead of venue and food.

Which form do you need?

The event evaluation form is one of the inspection, report & evaluation forms. If it is not quite the job, one of these may be.
