What this form asks, and why

Question Why it is there
What would you tell someone thinking of working with us? Required The quote itself. Asking what they would tell someone deciding gets a usable sentence rather than “great service”.
What did we do for you? (optional) Optional context, so the quote is never shown beside work it was not about.
Your name Required Kept even when it is not shown: your record of who said it.
Email Required Never published. It lets you check a shortened version with the writer.
How should we show your name? Required Four choices, from full name to none, so people who would refuse outright can still say yes.
Your role and company, if you’d like them shown (optional) Only when “How should we show your name?” is not Don’t show my name Only asked when a name will be shown. A role and company give a business quote its weight.

What we left out

  • A star rating. Stars beside hand-picked quotes suggest an overall score your selection does not support.
  • A reward for writing one. A reward has to be disclosed beside the quote and can never depend on praise (see the FTC note below), so the form offers none.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

Make it yours

  1. Name yourself in the permission

    Replace [Business name] and the places you publish in the permission tick, and the take-down [contact] in the note below it. The permission covers only what it names.

  2. Know which job it is about

    Send each client a link with ?job=Kitchen renovation. The hidden job field saves it with their quote.

  3. Keep quotes and permissions together

    Connect → Google Sheets or Notion adds each testimonial with its display choice and the time permission was given.

Questions about this template

Can I edit a testimonial before I publish it?

Only if they ticked the editing box, and then only to shorten it or fix spelling. Send them the shortened version first: the thank-you screen can promise that.

Can customers give a testimonial without their name on it?

Yes. “Don’t show my name” publishes the quote without one, while you keep the name and email privately as evidence it is genuine.

Is this the same as a testimonial release form?

It records permission to publish a written quote. For video, photos in advertising or a paid endorsement, use a signed release as well.

What if someone asks for their testimonial to be removed?

Take it down from wherever you published it. Permission can be withdrawn, and the note at the end of the form says who to ask.
