What we left out
- Home address. 5 of 11 forms ask for one. The email and signature prove consent without it.
- Ethnicity. 1 of 11 forms asks, a stock agency, for keywording. It is sensitive data a photographer does not need.
- Bank details for the fee. The form records the amount. Bank and card numbers never belong in a form field.

In September 2026 we read 11 model release forms that organisations use today (US 8, UK 2, Australia 1) and recorded every field. This is how often each one appears. It is part of the Form Field Index.

Sign on set
Print the QR code from the Share tab and tape it by the door. Models sign on their own phones between looks.

Keep it off the open web
Settings → Password protection means only people you give the password to can open the release.
Good practice and the law
This is general information, not legal advice. Rules differ by country and state, so check anything important where you operate.

In New York, using a person’s name, picture or voice for advertising or trade needs their written consent, or a parent’s for a child. New York Civil Rights Law §50

Questions about this template
Do I need a model release if the model is not paid?
Payment does not decide it; the use does. Choose “No payment” and the release says so plainly instead of hiding it in “valuable consideration”.
Do models sign a new release for every shoot?
It is cleaner. The release names the shoot, so a new shoot means a new response, grouped by its own link.
Can a model under 18 sign?
Not alone. Answer No to “Are you 18 or older?” and the form asks for a parent or guardian, who signs.
Is this a 2257 release?
No. US producers of sexually explicit material must check and keep each performer’s ID, and this form does neither.
Which form do you need?
The model release form is one of the consent & release forms. If it is not quite the job, one of these may be.
