What we left out
- An indemnity. 10 of 13 forms make the participant indemnify the organisation, and 4 of 13 forms add its legal fees. That goes beyond giving up a claim; add it only on legal advice.
- A witness or notary. 3 of 13 forms want a witness, and a hotel’s is notarised. An online form cannot notarise; the response records who signed and when.
- Governing law and venue. 5 of 13 forms name one. Add yours if your lawyer wants it.

Questions about this template
Is a liability waiver legally binding?
Often, not always. Courts read waivers strictly and some liability cannot be waived at all, such as a UK business’s for negligent injury. Have a lawyer check the release where you operate.
What is the difference between a waiver and an indemnity?
A waiver gives up the participant’s own claims. An indemnity makes them cover the organisation’s costs when someone else claims. This form has the first, not the second.
Can a parent sign a waiver for a child?
They can sign it, and the form asks for a parent when the participant is under 18. Whether a child is held to it varies by state and country.
Does a waiver need signing every visit?
Not always. 2 of 13 forms say how long theirs lasts, from one event to every visit in 12 months. Say yours in the first paragraph.
