What we left out

Good practice and the law

This is general information, not legal advice. Rules differ by country and state, so check anything important where you operate.

Questions about this template

Can a client sign a tattoo consent form while drunk?

They should not be tattooed. The form has them declare they are sober; the artist still decides.

Does it have to be notarised?

Not for an adult. Florida needs a parent’s notarised consent at 16 or 17, and an online form cannot notarise.

Is it covered by HIPAA?

No. A studio is generally not a covered entity, and PulseFormCreator is not HIPAA compliant. Treat it as a consent record, not a medical record.

Do I need to keep a copy of the client’s ID?

Usually not: see it, then record the ID type in the studio section.
