What happens after someone submits

Set it up once in the Workflows tab, and every consent is on file before the needle starts.

  2. A client signs the consent

    The studio gets the answers by email, with a PDF of the signed consent to keep for inspections.

  3. A health answer is Yes

    Reply from their response in Results to ask about it, and their answer comes back into the same thread.

  4. They finish the form

    The thank-you screen can offer a Book a time button for the touch-up, with their name and email filled in.

  5. The client comes back

    Their People record shows every consent they have signed, free for up to 1,000 people.

What this form asks, and why

A tattoo consent form records who is tattooed and their age, the design and where it goes, a health screen, sobriety, the risks, permanence and aftercare, then signatures. Florida’s rules set a minimum record: the artist, the client’s name, age and birthdate, the tattoo and its placement, both signatures and the date.

The last column shows how often real tattoo consent forms ask for it.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When any Yes to a health question

    shows “Tell us about your Yes answers, and any medicine that affects healing”

  2. When “Have you put numbing cream on today?” is Yes

    shows “Which cream?”

  3. When the link’s mode value is “studio”

    shows “For the studio”, “Photo ID seen” and “Artist signature”

  4. When “Are you 18 or over?” is No

    shows the “We can’t tattoo anyone under 18” ending

  5. When any Yes to a health question

    shows the “Thanks Jordan, your artist will talk your answers through first” ending

Make it yours

  1. One link, two screens

    Clients use the plain link. On the studio tablet, ?mode=studio adds the ID check and artist signature.

  2. Preselect the artist

    Add ?artist= and a name to each artist’s booking link, and the box fills itself in.

  3. Sharing one tablet?

    Turn off Settings → Responses → “Save respondents’ progress on their device”, so a half-finished form is not waiting for the next client.

This is general information, not legal advice. Rules differ by country and state, so check anything important where you operate.

Questions about this template

Can a client sign a tattoo consent form while drunk?

They should not be tattooed. The form has them declare they are sober; the artist still decides.

Does it have to be notarized?

Not for an adult. Florida’s parent consent at 16 or 17 is notarized on its own form, DH 4146.

Is it covered by HIPAA?

No. A studio is generally not a covered entity, and PulseFormCreator is not HIPAA compliant, so treat this as a consent record.

How long should I keep signed consent forms?

As long as your state or county says. Florida’s rules say two years, parental consents included. Download any one as a PDF of its response.
