Make it yours

  1. Send it with the booking

    Paste the link into your booking confirmation. Add ?therapist= and a name, and it is stored with the response as a hidden field.

  2. Know before they arrive

    Turn on Email me in the Connect tab, so a retinoid or Accutane Yes reaches the therapist before the appointment.

  3. Use your own waiting period

    Salons we read wait from 2 to 12 months after Accutane. Edit the question and the “We can’t wax you just yet” ending to match yours.

Good practice and the law

This is general information, not legal advice. Rules differ by country and state, so check anything important where you operate.

  • Everywhere

    Avoid waxing during isotretinoin treatment and for 6 months after, because it raises the chance of scarring. MedlinePlus, isotretinoin

  • Australia

    NSW Health treats waxing as skin penetration: register with your council, use single-use spatulas and never reuse wax on another client. NSW Health, waxing

  • US

    HIPAA covers health plans, clearinghouses and providers that bill electronically. A wax studio is generally none of these. CMS, HIPAA covered entities

  • UK

    Health answers are special category data: a salon needs a lawful basis and an Article 9 condition. ICO, special category data

Questions about this template

How long after Accutane can a client be waxed?

MedlinePlus says 6 months after the last dose. The form asks about the last 6 months and ends early for a Yes.

Do clients fill it in every visit?

No. The last consent line asks them to tell you if their medication or skin changes; then send the link again.

Can someone under 18 be waxed?

That is your salon’s call. The form asks, and a Yes adds a parent or guardian’s name above the signature.

Is a waxing form a medical record?

No. It is the salon’s screening and consent record, and PulseFormCreator is not HIPAA compliant, so keep clinical notes elsewhere.
