What we left out
- Medical insurance and policy number. Not needed for a day trip, and it turns the slip into a health record. PulseFormCreator is not HIPAA compliant.
- Date of birth and home address. The school already has both.
- A parent indemnity. It alarms parents, and in California the waiver for school trips is already in the Education Code.
- Payment. Put the cost in the trip details with a link to the school’s payment system. The form takes no payments.

Good practice and the law

In England, schools always need written consent for nursery-age children; for older pupils, for higher-risk trips and trips outside school hours. DfE, health and safety on educational visits
- US
In California, everyone on a school field trip is deemed to have waived claims against the district, and no pupil may be kept off a trip for lack of money. California Education Code §35330
- Australia
Victorian government schools need written or electronic consent for day excursions, overnight stays and adventure activities, and must tell parents the activities, risks, transport and supervision. Victorian Department of Education, excursions
Questions about this template
Can parents sign a permission slip online?
Yes. They draw their signature on screen from the link in your class message, and each response is stamped with the time it was sent.
Can a student sign their own permission slip at 18?
At 18 they are an adult, so usually yes, but your school’s policy decides. Duplicate the form and change the parent questions to “Your name” and “Your phone”.
What happens when a parent says no?
They see a screen saying their child will stay at school, and you have the answer in writing instead of a missing slip.
Does one slip cover the whole year?
Use one per trip, so parents know what they are agreeing to. English and Victorian guidance allow one consent for routine local visits, with notice before each.
