What we left out

Card number and CVC. Seen on 1 of 15 forms. Card numbers have no place in a registration you export and share.
Health insurance and family doctor. Seen on 3 of 15 forms. Counsellors never need them.

Questions about this template

Can I register two children on one form?

In the VBS preset, yes: “Add another child?” opens a block per child. Camp keeps one child per form, because weeks and health differ.

Who is allowed to collect a child?

Whoever the parent lists. Put your own rule, such as adults over 16 with photo ID, in the question’s description.

Should the health history go on this form?

No. Keep allergies, conditions and medications here and send your camp’s own health form separately. The sleepaway preset links to it.

Can families pay when they register?

No. PulseFormCreator does not take payments. The fees paragraph states the amounts and links your payment page.
