This registration form template signs people up for a class, club or program and collects what you need to confirm their place: contact details, the program they chose, an emergency contact and consent. Guardian questions only appear for under-18s. Set a response limit and the form closes when the program is full.

Why this template works
- Asks for guardian details only when the person is under 18, so adults see a shorter form.
- Closes at the response limit you set and shows a "program is full" message that points to a waitlist.

Questions about this template
Can I limit the number of registrations?
Yes. Set a response limit in Settings. When it is reached, the form shows your "program is full" message instead of the questions.
Can people pay when they register?
No. PulseFormCreator does not take payments. Put bank-transfer details or a link to your payment or ticketing page on the confirmation screen, which you write yourself.
Can I ask different questions for under-18s?
The template already does. A yes or no question asks whether the person attending is under 18. If yes, the parent or guardian name and mobile appear and are required. Everyone else skips them.
Do registrants get a confirmation?
Everyone sees a confirmation screen that you write. It can include their first name and email address, what to bring and a button back to your site. A confirmation email to the registrant is on the Pro plan.
Can one form cover several classes?
Yes. List the classes in the program dropdown, then link each class from your site with ?program= and its name. The form opens with that class chosen, and the results show which one each person picked.
Does it cover Australian privacy and consent rules?
It opens with a short collection notice with space for your privacy policy link, which is what Australian Privacy Principle 5 asks for. The marketing box starts unticked and mentions unsubscribing, in line with the Spam Act 2003. Check the wording for your organisation. This is not legal advice.
