What this form asks, and why

A class registration form needs the class, the participant’s name and date of birth, a parent for anyone under 18, contact details, an emergency contact, health or access needs, and agreement to your terms. 13 of 23 forms ask for an emergency contact and 19 of 23 forms end with a signature.

The last column shows how often real registration forms ask for it.

What we left out

Card numbers. 4 of 23 forms ask for one. A form field is no place for card data.
Gender. 13 of 23 forms ask it. Add it back only if your divisions are split by gender, with a “prefer not to say” option.
Office use only. 9 of 23 forms keep a box for staff. Your responses table does that job.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

Close it at capacity

Set Settings → Limit responses to your class size, and the form shows “This class is full” once it is. Put your waitlist email in that message.
Give each class its own link

Add ?program=Beginners – Mondays 6 pm to the link on each class’s page and the form opens with that class chosen.
Close on the enrollment deadline

Set a close time under Settings → Schedule. Anyone who arrives later sees “Registrations for this class have closed.”

Questions about this template

Can I take the class fee on the form?

No, payments are not available. Put the price in each class option and your payment link or bank details on the thank-you screen.

How do parents sign up brothers and sisters?

One form per child. Point the thank-you button at the form’s own link and the next one is a tap away.

Does the signature need a date beside it?

No. Every response is stamped with the time it was sent, and that shows in your results and export.

Can I use it for a sports season or for lessons?

Yes. Pick “Youth sports season” or “Swim, dance or music lessons” above the preview. Only the questions that differ change. Order uniforms on a team shirt order form.

Which form do you need?
