What’s inside the membership application form template

This membership application form takes new members and yearly renewals for a club or association. It collects applicant details, the membership type they want, what they would like to be part of, agreement to the club rules and a signature. No payment is taken: the last screen tells the applicant an invoice follows once the committee approves them.

Why this template works

  • Each membership type opens its own question: family lists who it covers, concession asks which card, and junior asks for a parent or guardian.
  • One link serves joining and renewing. Pick "Renewing my membership" and the member number question appears.
  • Two required boxes cover the club rules and the code of conduct, and a signature question captures the applicant's signature by mouse, finger or typed name.

Questions about this template

Can the form take the membership fee?

No. PulseFormCreator has no payment question, so nothing is charged here. The form says so on the way through, and the thank-you screen tells the applicant you will email an invoice with your bank details and the due date once the committee approves them.

Is the membership form free to use?

Yes. Unlimited members can apply on the Free plan, under the fair-use policy, and signatures, logic and consent boxes are included. Removing the PulseFormCreator badge is on the Pro plan.

Can members sign the application?

Yes. The signature question accepts a drawn signature on a phone or a typed name, and it is stored with the answers. The applicant also dates it. Ask your own committee whether that meets your rules.

Can I use it for yearly renewals?

Yes, that is what the first question is for. Send the same link each year with ?season=2027 on the end, and every renewal is tagged with the year in your results.

Where do applications go?

Into your PulseFormCreator dashboard, and out to CSV or Excel for your member register. Turn on "Email me" to get each one, or send them to Google Sheets, Airtable or Notion for the committee to review.

Can I change the membership types?

Yes. Edit the five options to match your rules, and change the follow-up question that appears with each one. Fees stay on your own website, since the form does not price anything.
