What this form asks, and why

Question Why it is there
Full name Required For the invoice and the attendance certificate.
Name on your badge Optional, so Elizabeth can be Liz on the lanyard while the invoice stays formal.
Email Required Confirmation, program changes and the certificate. It also blocks double registrations.
Mobile Optional, for the registration desk on the day.
Organisation Required Printed on the badge and the delegate list.
Job title Optional. A second badge line.
Registration type Required Conference fees depend on who you are, not on a ticket, so each rate carries its price.
Member number Required Only when “Registration type” is Member – $[price] Shown only for the member rate, so staff can check it before invoicing.

What we left out

  • Card number and security code. PulseFormCreator takes no payments, and card data does not belong in a form field. Invoice from your own system.
  • A fee grid with a total to add up. Prices sit in the registration options, and the total belongs on the invoice.
  • Signature and date. The cancellation tick records agreement, and every response is stamped with the time it was sent.

Make it yours

  1. Send members straight to their rate

    Link member mail-outs with ?type=member_price and the member rate is already chosen, with the member number question showing.

  2. Track group and sponsor codes

    Add ?code=GROUP10 to a link. The code is saved with each registration, ready to apply on the invoice.

  3. Close at capacity and at the deadline

    Set Settings → Limit responses to the venue size and a close time under Settings → Schedule. Each shows its own closed message.

Questions about this template

Do speakers fill in the same form?

Yes. “Speaker (no fee)” is a registration type, and speakers finish on their own thank-you screen with what happens next. Exhibitors get one too.

Can a manager register several staff at once?

Each delegate needs a badge, so each registers. Every registration can name the same accounts contact for the invoice.

Can I print name badges from the registrations?

Yes. Export to CSV or Excel: badge name, organisation and job title are separate columns. Where the badge name is blank, print the full name.

Can I use it for a symposium or a training course?

Pick “Academic conference or symposium” for paper titles and themes, or “Professional training course” for licence numbers and CPD records.

Which form do you need?
