What this form asks, and why

What we left out
- Student ID number. A name and a class are enough to hand shirts out.
- Home address and phone. Shirts go to class. Add an address only if you ship.
- A total for parents to work out. The form does the sum, surcharge and names included.

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

Give each class its own link
Add ?class=Year 3 – [teacher] to that class’s link, with your class name, and it is already chosen.

Close on the printer’s deadline
Set a close time under Settings → Schedule. Late parents see “Orders for this print run have closed.”

Cap the print run
If the printer limits the run, set Settings → Limit responses. Late parents see “This print run is full.”

Questions about this template
Can parents pay for the shirts on the form?
No. PulseFormCreator does not take payments. The form adds up what is owed and the thank-you screen says how to pay: an envelope, a transfer or your school’s payment link.
How do I add up the sizes for the printer?
Export the responses. Each size is a column, and its sum is the number to print.
Can I use it for a sports team?
Pick “Team” above the preview: player and team instead of student and class, and a name and number on every shirt.
What if a family wants shirts in two colours?
They send a second order. Each order then has one colour, so filtering the export by colour gives the printer a count per colour and size.
