What we left out
- Race or ethnicity. On 2 of 13 forms. Leave it out unless your award is lawfully restricted (see the IRS note below).
- Parents’ jobs and employers. On 3 of 13 forms. The finances question covers what matters, and a guardian’s email handles consent.

Questions about this template
Should a scholarship form ask about citizenship?
Only if the award is limited to citizens or residents; then add one yes/no question. The home address already covers awards for one town or region.
Can students apply before a college accepts them?
Yes. “Not yet” is an answer. Ask for proof of enrolment before you pay the award.
How do recommenders send their letters?
The student names one recommender and their email. Send that person a short reference form of your own, with ?student= and the applicant’s name in the link.
Can I run a need-based bursary with it?
Pick “Need-based award” above the preview: the finances question becomes required, and household size and other aid are added.
Which form do you need?
The scholarship application form is one of the application forms. If it is not quite the job, one of these may be.
