What’s inside the appointment request form template
This appointment request form asks for a preferred date and time and two alternatives, so you confirm one by email instead of handing over your diary. It also asks what the appointment is about, whether the person is new or returning, and where it should happen. Every screen says the same thing: nothing is booked until your confirmation email arrives.

Why this template works
- Three times, not one. The person gives a preferred slot and two alternatives, so most requests are settled in a single reply.
- Choose "Something that cannot wait" and the form shows your phone number and finishes on an urgent screen instead of the usual one.
- The address question only appears for a site visit or when you travel to them, so everyone else sees a shorter form.

Questions about this template
Does the form check my calendar or show my availability?
No. PulseFormCreator has no calendar integration and cannot see whether a time is free, so the form never shows available slots and never holds one. That is why it asks for three preferred times, and why every screen calls it a request. You confirm the time by email.
Is the appointment form free?
Yes. Unlimited requests, logic and both thank-you screens are on the Free plan, under the fair-use policy. You can build the form without an account and confirm your email when you publish it.
Does it send reminders or text messages?
No. PulseFormCreator sends no text messages and no reminders. You reply to the request from your own inbox, which is also where a change of time is easiest to handle.
Can I take a deposit or payment with the request?
No. There is no payment question. If you take a deposit, put your bank details or a link to your payment page on the thank-you screen, which you write yourself.
Can I put it on my website?
Yes. Publish it, open Share and copy an embed code: inline, pop-up, slider or side tab. It works on WordPress, Webflow, Squarespace, Wix and plain HTML, or you can share the hosted link and its QR code.
How do I know a request has come in?
Every request is saved in your dashboard. Turn on "Email me" to get each one in your inbox, or send them to Slack, Google Sheets or a webhook so nothing sits unanswered.
