What this form asks, and why

An inquiry form should ask what the inquiry is about before anything else, then only what that type needs: an order number for orders, an organization for partners, a deadline for press. Contact details come after the inquiry itself, so people start with what they came to say.

The last column shows how often real inquiry forms ask for it.

Question Why it is there Real forms
What is your inquiry about? Required 4 of 12 forms ask it, all of them government agencies. As on NSW Local Land Services’ form, each answer here opens its own questions. 4 of 12 forms
Which product or service? Only when “What is your inquiry about?” is “A product or service” Product inquiries only. 2 of 12 forms
When would you need it? Only when “What is your inquiry about?” is “A product or service” Tells a buyer from a browser without asking for a budget. Our own recommendation
Order or booking number Required Only when “What is your inquiry about?” is “An order or booking I have made” Order inquiries only. 3 of 12 forms ask for an existing reference, so the reply joins the right case. 3 of 12 forms
What do you need? Required Only when “What is your inquiry about?” is “An order or booking I have made” Change, cancel, a problem or timing: the answer decides who picks it up. 3 of 12 forms
Photos of the problem (optional) Only when “What do you need?” is “Something is wrong with it” Only for a problem with an order, where a photo settles it faster. 2 of 12 forms
Organization name Required Only when “What is your inquiry about?” is “Working with us or a partnership” Partnerships only: the organization matters more than the person. 5 of 12 forms
Organization website Only when “What is your inquiry about?” is “Working with us or a partnership” Optional, so you can look them up before replying. Our own recommendation
Publication or outlet Required Only when “What is your inquiry about?” is “Media or press” Press only: the outlet decides who answers and how fast. Our own recommendation
Your deadline Only when “What is your inquiry about?” is “Media or press” A press answer is only useful before the story runs. Our own recommendation
Tell us about your inquiry Required The inquiry in their words, asked by 10 of 12 forms. 10 of 12 forms
Your name Required Asked after the inquiry itself. 11 of 12 forms
Email Required Where the reply goes unless they ask for a call. 11 of 12 forms
How should we reply? Required 4 of 12 forms ask how to reply. Two choices: you will email or call. 4 of 12 forms
Best number to call Required Only when “How should we reply?” is “Call me” Shown, and required, only for people who asked for a call. 11 of 12 forms
Best time to call Only when “How should we reply?” is “Call me” So the call comes when they can take it. Our own recommendation

What we left out

  • Postal address. 6 of 12 forms ask for one. You need it only when the reply goes by mail.
  • A title. Mr, Ms or Dr appears on 1 of 12 forms. It reveals gender and marital status and changes nothing.
  • “How did you hear about us?”. Asked on 1 of 12 forms. The hidden utm_source field records where people came from instead.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When “What is your inquiry about?” is “A product or service”

shows “Which product or service?” and “When would you need it?”

  2. When “What is your inquiry about?” is “An order or booking I have made”

shows “Order or booking number” and “What do you need?”

  3. When “What do you need?” is “Something is wrong with it”

shows “Photos of the problem (optional)”

  4. When “What is your inquiry about?” is “Working with us or a partnership”

shows “Organization name” and “Organization website”

  5. When “What is your inquiry about?” is “Media or press”

shows “Publication or outlet” and “Your deadline”

  6. When “How should we reply?” is “Call me”

shows “Best number to call” and “Best time to call”

  7. When “What is your inquiry about?” is “Media or press”

    shows the “Thanks, Tom” ending

  8. At the end

shows one of 2 thank-you screens, each headed “Thanks, Tom” with its own message

Make it yours

  1. Use your own inquiry types

Rename the options in the first question. To give a new type its own questions, add them and use “Only show this question when…” on each.

  2. Preselect the type from each page

Link your press page to the form with ?enquiry_type=Media or press and it opens with that type chosen.

  3. Keep a log each team can filter

Send inquiries to Google Sheets from the Workflows tab: the type gets its own column to filter by.

Good practice and the law

  • UK

Say why you collect details when you collect them: the privacy line does this once you add your policy link. ICO, right to be informed

  • Australia

APP 5 asks you to say who you usually pass details to. If a contractor handles some inquiry types, name them in the privacy line. OAIC, APP 5

Questions about this template

What is an inquiry form, and why not just list an email address?

It is a contact form that asks what the inquiry is about, so each one arrives with the order number, deadline or organization the right person needs, not as a bare email in a shared inbox. Embed it on your site or share its link. For booking a time, use an appointment request form.

How fast should I answer an inquiry?

As fast as your thank-you screens promise: change one business day, and two hours for press, to times you can keep. The daily digest counts inquiries still waiting on your reply.

Can each inquiry type go to a different email address?

Yes. Add a custom workflow for each team: it checks the answer and emails them. Three are free per form, more on Pro.

Is an inquiry form the same as an enquiry form?

Yes. Enquiry is the British and Australian spelling. This template says inquiry; change the wording in the editor if your readers expect the other.

Which form do you need?

Not quite the job? One of these may fit, or browse all contact and feedback forms.
