What we left out
- Card details for the stall fee. Taken inside 1 of 12 forms. PulseFormCreator takes no payments: invoice accepted stallholders, so nobody who misses out needs a refund.
- A Social Security number as the tax ID. On 1 of 12 forms. Ask for a business or sales tax number, or nothing.
- Food permits from every vendor. On 4 of 12 forms, always in a food section. The food preset asks; the craft form does not.

Add ?event=Spring Twilight Market to the link. The heading names the event, and it is saved with each application.

Close on your cut-off date
Set the close time under Settings → Schedule; late applicants see “Stall applications for this event have closed.”

In the Connect tab, send each application to Google Sheets, then sort by category, stall size and power.
Questions about this template
How do I choose between vendors?
Publish your criteria at the top of the form (handmade, local, a mix of categories), then compare applications by category with the photos side by side.
Can a stallholder apply for only some market days?
Yes: they tick the dates they can do. The CSV or Excel export gives each date its own 1/0 column, so you can count stalls per day.
Can stallholders send insurance later?
Yes. The certificate upload is optional. Make it required if your venue will not book an uninsured stall.
Can food trucks use the same form?
Pick “Food vendor or food truck” above the preview for the kitchen, permit, vehicle length and generator questions.
Which form do you need?
The vendor application form is one of the application forms. If it is not quite the job, one of these may be.
