How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When “Are you under 18?” is Yes

    shows “Parent or guardian email”

  2. When “Are you under 18?” is Yes

shows the “Thanks, Maya. One more step.” ending

Make it yours

  1. One form for every fund

Add ?award=Smith Memorial Scholarship to each award’s link. The heading names it, and it is saved with the application.

  2. Close on your deadline

Set the close time under Settings → Schedule; late visitors see “Applications for this round have closed.”

  3. Score it with the panel

Export the responses to Excel from Results and give each judge a column per criterion.

Good practice and the law

Questions about this template

Should a scholarship form ask about citizenship?

Only if the award is limited to citizens or residents; then add one yes/no question. The home address already covers awards for one town or region.

Can students apply before a college accepts them?

Yes. “Not yet” is an answer. Ask for proof of enrollment before you pay the award.

How do recommenders send their letters?

The student names a recommender and can attach a letter they already have. Otherwise send that person a short reference form, with ?student= and the applicant’s name in the link.

Can I run a need-based award with it?

Pick “Need-based award” above the preview: the finances question becomes required, and household size and other aid are added.

Is a scholarship the same as an educational grant?

Close, but the forms differ. A scholarship usually rewards merit or need; a grant or bursary often pays for a set cost. “Educational grant or bursary” asks for the purpose, each cost and the total instead of grades and a transcript.

Which form do you need?

Not quite the job? One of these may fit, or browse all application forms.
