What we left out

  • Bank account numbers. On 10 of 22 forms. A bank reference needs only the bank and branch, and account numbers do not belong in a form field.
  • Directors’ dates of birth and licence numbers. On 9 of 22 forms and 7 of 22 forms, all Australian. Ask a guarantor for them only if you run a consumer credit check.
  • Spouse details. On 2 of 22 forms. A guarantee binds the person who signs it; in the US, Regulation B limits requiring a spouse’s signature (below).

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

Make it yours

  1. Set your own threshold

    Change the $20,000 in the financial statements question’s “Only show it when earlier answers match” condition, and in its description.

  2. Know which rep sent it

    Add ?rep=Sam to the link you email a new customer, and the rep’s name is saved with the application.

  3. Send it straight to accounts

    In the Connect tab, turn on Email me and add your accounts address under “Also send to”, or post each application to your credit team’s Slack channel.

Good practice and the law

  • US

    You may ask for a guarantor, but may not require a spouse’s signature when the applicant qualifies alone. CFPB, Regulation B §1002.7

Questions about this template

What is a credit application form for a business?

The form a supplier sends before letting a business buy now and pay later, often in 30 days. It gathers what the supplier needs to set a limit, and to recover a debt.

Where do I record the limit I approve?

Some paper forms end with an office-use box. Here, add Approved limit and Account number columns to the synced Google Sheet: new applications keep landing in the form’s own columns.

Why don’t sole traders see the guarantee?

Sole traders and partners are already personally liable for the business’s debts, so the guarantee appears only for companies and trusts.

Who should a customer give as trade references?

Suppliers who give them credit now, ideally for similar amounts. Friends, relatives and their own accountant tell you nothing about how they pay.
