What this form asks, and why

A product order form lists each item with its price and a box for the quantity, then asks pickup or delivery, a date, the buyer’s name, email and phone, and how they will pay. 89% of real order forms have a quantity box and 75% a total line.

The last column shows how often real order forms ask for it.

What we left out

  • Card number and security code. 12 of 28 forms take one on paper. Card data does not belong in a form field; the thank-you screen says how to pay instead.
  • A signature. On 11 of 28 forms, mostly to authorize a card charge. No card, nothing to sign.
  • What to do if an item sells out. Every form that asks sells seeds or plants. Add a single choice if your stock runs out mid-season.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When “Soy candle – $28” is 1 or more

    shows “Candle scent”

  2. When “Reed diffuser – $35” is 1 or more

    shows “Diffuser scent”

  3. When “Pickup or delivery?” is “Delivery (+$12)”

    shows “Delivery address”

  4. When the order total is 0 and “Anything not listed? Item name or code, and how many” is left blank

shows the “Your order is empty” ending

  5. When “Pickup or delivery?” is “Pickup”

shows the “Thanks, Tolu. Your order total is $169.” ending

  6. As people answer

    adds up the order total

Make it yours

  1. Stop when you sell out

Set Settings → Limit responses to the orders you can fill, and a close time under Settings → Schedule.

  2. See which flyer sold

Add ?source=market-stall to each link you share. It is saved with every order.

  3. Give a product its own option

Copy the scent question, change its choices, and show it only when that product’s quantity is 1 or more.

Good practice and the law

  • US

Ship within the time you state, or 30 days if you state none, unless the buyer agrees to a delay. FTC, Mail Order Rule

  • Australia

Show the total price as one figure. A delivery fee may sit outside it only when free pickup is offered, as here. ACCC, displaying prices

Questions about this template

Can a buyer choose a different scent for each candle?

Yes. They pick “A mix” and say which in the notes, so one scent question per product keeps the form short.

Does the running total include delivery?

The first page totals the products and says delivery adds $12. Choose delivery and the form adds a shipping address question; the thank-you screen shows the total with the fee.

What if someone submits without ordering anything?

If every quantity is zero and nothing is written in, the form stops and asks them to add an item.

Can it take orders from a long catalog?

Yes. List your best sellers with quantities; buyers write the rest by name or code, and you price those when you confirm. For menus, custom cakes, school fundraisers or shirts by size, see the other order form templates.

Do the prices include tax?

Pick “United States” above the preview for a “plus sales tax” line, or “Australia” for prices that include tax, shown as one figure.

Which form do you need?

Not quite the job? One of these may fit, or browse all order forms.
