What we left out
- A signature. 12 of 15 forms have one, because they change an account or a legal record. A guest list is neither.
- ID numbers and date of birth. 3 of 15 forms ask for a government ID number and 3 of 15 forms for a birth date, just to change where post goes. Nobody needs either to post a card.
- The old address. 6 of 15 forms ask for it because they update a record. A new list has nothing to update.

Set your country
Pick the address question’s Default country in its settings. People abroad can still choose theirs, and the state and postcode boxes adjust.

One link per side of the family
Add ?list=Work friends or ?list=Mum’s side to each link you send. The hidden list field records it, so you can sort the replies later.

Pick the preset
“Directory or class list” adds a former name and a sharing question; “Shipping details” makes phone required and asks for delivery instructions.

Close it when the envelopes go out
Turn on Settings → Schedule and set it to close on your posting date.

Questions about this template
Where do I get a change of address form for the post office or the IRS?
From them: USPS, the IRS (Form 8822) and your motor vehicle office each have their own. This template collects other people’s addresses.
Can I print envelope labels from the responses?
Yes, from the CSV or Excel export: one row per response, the envelope name in one cell and the whole address on one line in the next (“14 Harrow Street, Unit 3, Brunswick VIC 3056, Australia”). If your label tool wants street, suburb and postcode in separate columns, split that cell first.
Does it work for addresses outside Australia?
Yes. People pick their country on the form; outside Australia the state box becomes free text and the postcode takes letters.
Can a family send one address for everyone?
Yes. That is what the envelope name is for: “The Nguyen family” is one response and one envelope.
