What this form asks, and why

An RSVP form needs the guest’s name, a yes or no, how many are coming, the names of the others and any dietary needs. Ask the yes or no first: a no is an answer the host needs too, and guests who decline should not wade through meal questions.

What we left out

Postal address. A reply needs none. Collect addresses before the invitations go out, with an address form.
A separate “bringing anyone?” question. The headcount does both jobs: one means just you.
Phone as a required field. Most hosts only need email, or nothing. Add a phone question if you plan to text reminders.

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

Make it yours

Send each guest a personal link

Add ?guest=Hannah Brooks to the link and her name is already filled in. Add ?event=Sam’s 30th and the invitation line changes to match.
Cap the seats on each invitation

Add ?seats=2 to a guest’s link and the headcount will not accept more than two. Leave it off and the form allows up to ten.
Close on your reply-by date

Set the close time under Settings → Schedule. Late guests see “RSVPs are closed. Please contact the host directly.”
Keep the guest list

Connect Google Sheets and each reply becomes a row: name, answer, headcount and dietary needs. Paste your invitation list beside it and the gaps are the guests who have not replied. The form cannot remind guests who never opened the link, and a sent reply cannot be edited, so changes come to you. Free has no reply cap.

Questions about this template

What does RSVP stand for?

Répondez s’il vous plaît, French for “please reply”. It asks guests to answer either way by a date, which is why this form starts with yes or no.

Can a guest change their reply?

Not by replying again: the form takes one reply per browser and asks a second try to contact you. Turn off Prevent duplicates in Settings to accept a new reply, then keep the later row.

Can I ask for meal choices for a wedding?

Yes. Pick “Wedding” above the preview for a wedding RSVP form with a name and a main course for each guest, so meals export one column per guest, plus a song request for the band.

What do guests who can’t come see?

Only the note for the host. Then the form thanks them by name, and the reply still lands in your list as a no.
