What this form asks, and why
A cake order form asks the date and time, pickup or delivery, the shape and the size by servings, the flavor, filling and frosting, the design and the message, then any allergy before you quote. Among bakery forms, 9 of 10 forms ask the size and 9 of 10 forms the date.

Match your price list
Change the sizes and flavors, then the numbers in the “Starting price” rules under the size question.

Offer gluten-free and vegan cakes
Add them to the flavor list and the “Premium flavor” rule, for example $10 more for gluten-free.

Questions about this template
How do customers pay the deposit?
After you confirm the price: the thank-you screen explains the deposit and how to pay it. Nothing is charged on the form.
Is the price on the thank-you screen final?
It is a starting price from the size, flavour and delivery. You quote the decoration and confirm by email.
What happens when someone picks Wedding?
The form ends there, before any cake questions, with your booking link and phone number. Tiered cakes need a tasting and a conversation.
How do I ask for a week’s notice?
A date question takes a fixed earliest date, not a number of days ahead, so the notice goes in the intro and the date’s description.
