  2. A participant signs

You get the waiver by email, with a PDF of the signed waiver attached to keep.

  3. Before the session starts

Responses go to Google Sheets, so staff can check off who has signed at the door.

  4. They book again next season

Their People record shows each waiver they have signed, free for up to 1,000 people.

What this form asks, and why

A liability waiver names the parties and the activity, spells out its real risks, has the participant accept them, and gives the release in return for taking part. It also covers emergency treatment, brings in a parent for anyone under 18, and ends with a signature and the date.

The last column shows how often real liability waiver forms ask for it.

  • A witness or notary. 3 of 13 forms want a witness, and one is notarized. An online form cannot notarize; the response records who signed and when.
  • Governing law and venue. 5 of 13 forms name one. Add yours if your lawyer wants it.

shows “Parent or guardian’s name” and “Relationship to the participant”

Make it yours

  1. One link per session

Add ?activity=Saturday kayak tour to the link and the activity fills itself in.

  2. Cap the numbers

Settings → Limit responses to the places available, with your own “this session is full” message.

  3. Close the link after the session

Settings → Schedule stops new signatures once the session has passed, with your own closed message.

Questions about this template

Will a court enforce a liability waiver?

Often, not always. Courts read waivers strictly, and some liability cannot be waived at all, as the notes above show. Have a lawyer check yours.

How long should we keep signed waivers?

Until an injury claim would be too late, which for a child can mean years after they turn 18. Download any waiver as a PDF from its response.

Can a parent sign a waiver for a child?

They can sign it, and the form asks for a parent when the participant is under 18. Whether a child is held to it varies by state and country.

Does a waiver need signing every visit?

Not always. 2 of 13 forms say how long theirs lasts, from one event to every visit in 12 months. Say yours in the first paragraph.

Which form do you need?

Not quite the job? One of these may fit, or browse all release and consent forms.

  • Tattoo waiver form

For a tattoo, where informed consent and a health check matter more than a release.

  • Picture release form

When all you need is permission to use people’s photos.

  • Permission slip

For a school trip, where a parent gives permission rather than giving up claims.
