What happens after someone submits

Every lead is scored, saved and followed up, with no spreadsheet in between.

  1. Submitted

  2. A ready buyer finishes

A Book a time button opens your booking page with their name and email already filled in.

  3. The lead comes in

AI triage sets its priority and sums up what they need in one line.

  4. Straight after

The lead lands in People, with their answers and every email and reply on one timeline.

  5. Over the next week Pro

Three short follow-up emails go out, and they stop as soon as the lead replies.

What this form asks, and why

Only what decides the next step: who they are, what they need and how soon. A name, an email and the need are standard; only 3 of 11 forms ask a budget and 2 of 11 forms ask team size. Ours adds just the three questions that score.

The last column shows how often real lead generation forms ask for it.

Question Why it is there Real forms
Your name Required Asked on 11 of 11 forms. The first name comes back in the next question and on the thank-you screen. 11 of 11 forms
Work email Required Labeled “work email”, as on 3 of 11 forms, but free addresses go through: a founder on Gmail is still a lead. 3 of 11 forms
Company Required Required, and asked on 8 of 11 forms: a demo is for a company, not a person. 8 of 11 forms
Company website (optional) Optional, and asked on 3 of 11 forms. It saves sales a search before the call. 3 of 11 forms
How many people would use it? Required Scores the lead and decides whether budget is asked. It is the quickest sign of fit. 2 of 11 forms
What should the demo cover, Hannah? Required The question every form shares: what the demo should cover. 11 of 11 forms
When do you want to start? Required Scores urgency. With budget, it decides who sees your booking link. 2 of 11 forms
Do you have a budget in mind? Only when “How many people would use it?” is “2–10”, “11–50”, “51–200” or “201+” Optional, and hidden for one-person teams. The forms that ask it are all agencies. 3 of 11 forms
Phone (optional) Asked on 8 of 11 forms, most of them required. Optional here: a required number turns away people who only want to read. 8 of 11 forms
How did you hear about us? Asked on 5 of 11 forms. “An AI assistant” is an option because 10up’s list now names ChatGPT and Claude. 5 of 11 forms
Product news Unchecked and separate from the privacy line, where 10up bundles the two into one box. 3 of 11 forms

What we left out

  • Revenue and industry. Industry appears on 2 of 11 forms and revenue on 1 of 11 forms. Sales can read both from the website.
  • A calendar inside the form. Apptivo books the slot inside its form. Here only ready leads see your booking link, on the thank-you screen.
  • Job title. Asked on 1 of 11 forms. What they want the demo to cover tells you more.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When “How many people would use it?” is “2–10”, “11–50”, “51–200” or “201+”

shows “Do you have a budget in mind?”

  2. When “When do you want to start?” is “This month” or “In 1–3 months” and “Do you have a budget in mind?” is not “Under $1,000”

shows the “Let’s find a time, Hannah” ending

Make it yours

  1. Offer ready leads a booking link

Add your Calendly, Cal.com or HubSpot link to the “Book a demo” button. Only leads who need it soon and have a budget see it.

  2. Keep the campaign with the lead

The utm_source, utm_medium, utm_campaign and gclid hidden fields fill from your ad links and are saved with every lead.

  3. Create the contact in your CRM

In the Workflows tab, Zapier, Make or a webhook adds each request to HubSpot, Pipedrive or Salesforce as it arrives.

  4. Pick the version that fits

Choose “Agency or consultancy project” for a project brief, “Download or lead magnet” to trade a guide for an email, or “Local service” for the job, a ZIP or postal code and a call back.

Questions about this template

How is a lead generation form different from a contact form?

A contact form takes any message. A lead form asks what sales needs before the first call, scores the answers and offers ready buyers a booking link. For plain questions, use the contact us form.

Should the form reject Gmail and Outlook addresses?

We would not. Freelancers and founders on free addresses still buy, so this form lets them through.

What does the lead score add up?

Team size, timing and budget score 0 to 4 each, so a lead scores 0 to 12, saved with the request as Lead score.

What happens to someone who is just researching?

They finish on the second thank-you screen, with your guide instead of a booking link, and the request still reaches you.

Which form do you need?

Not quite the job? One of these may fit, or browse all contact and feedback forms.
