Set these up in the Workflows tab before the sellers share their links.

A supporter orders
It lands in Google Sheets with the seller’s name and class, so each product’s column adds up to the supplier order.
-
Each week of the sale
A weekly digest lists the new orders, so you can nudge quiet sellers before orders close.
-
A supporter asks about the next sale
They land in People with that answer kept, so next year’s sale starts with one email to them.
What this form asks, and why
A fundraiser order form names the seller, lists each product with its price and a quantity, adds up the total with any donation, and says how the order reaches the supporter and how they pay the group. The seller’s paper version adds a row per customer and a paid column.
| Question | Why it is there |
|---|---|
| Who is selling to you? Required | Each sale is credited to a seller, who also delivers it, so the seller comes first. |
| Chocolate chip cookie dough, 1.5 kg tub – $25 | Item and price in one label, like the numbered lines in a supplier’s brochure. |
| Fresh Christmas wreath, 50 cm – $45 | Each quantity feeds the running total, so nobody adds up a column by hand. |
| Soy candle, 300 g – $25 | Swap in what your supplier sells, and its price in the “Add up the order” rule. |
| Gift wrap, 3 rolls – $15 | Delete any line you do not sell. |
| Add a donation (optional) | For supporters who want to give without buying. It adds to the total. |
| Your name Required | The name the seller writes on the bag at hand-out. |
| Phone Required | So the seller can arrange the handover. |
| Email (optional) | Optional, since the seller already has the phone number. |
| How will you get your order? Required | Products arrive at the school or club, then go out through the sellers. |
| How will you pay? Required | The method only. Cash choices end on a screen saying how much to have ready. |
| Anything else? | A gift note, or “leave it on the porch”. |
| Future fundraisers | Unchecked, and kept apart from the order. |
What we left out
- Carbon copies. Paper sheets print copies for the supplier, the coordinator and the seller. Every order is already in your responses.
- The seller’s home address. Some supplier sheets ask it. Products go to the school or club, so the seller’s class or team is enough.
- A paid column. Paper sheets keep one per customer. Supporters ordering online choose how they will pay instead.
How the form changes as people answer
Most people see a shorter form. These questions and screens only appear when an answer calls for them.
-
When the order total is 0
shows the “Nothing ordered yet” ending
-
When “How will you pay?” is “Cash to my seller now” or “Cash to my seller when my order arrives”
shows the “Thank you, Margaret! Please have $120 ready for Ava Nguyen.” ending

Make it yours
-
Give every seller their own link
Add ?seller=Ava Nguyen&class=Room 12 to each seller’s link. Their name is filled in and the class is saved with every order.
-
Close on your order deadline
Set a close time under Settings → Schedule, so the supplier order goes in on time.
-
Stop at what you can supply
Set Settings → Limit responses if stock is capped. Late supporters see “We have sold everything we can supply.”
Good practice and the law
- US
A charity must give a written statement to anyone who pays more than $75 partly as a gift, such as an order plus a donation: the goods’ estimated value, and that only the rest is deductible. IRS, Publication 1771
- UK
Charities cannot claim Gift Aid on money paid for the products themselves. GOV.UK, what you can claim Gift Aid on
Questions about this template
How do supporters pay?
The form adds up the order, then the thank-you screen gives your group’s bank details or payment link, or tells cash payers what to have ready. Nothing is charged on the form.
How do I see what each seller sold?
Every order carries the seller’s name, and the export has a column for it: sort by seller for prizes and hand-out bags.
Can a seller enter orders taken in person?
Pick “Seller’s order sheet” above the preview: the customer’s name and phone, what they ordered and whether they have paid, one customer per form. On paper, use the seller tally sheet.
Can people donate without buying anything?
Yes. Leave every item at zero and enter a donation; the total is the donation.
Which form do you need?
Not quite the job? One of these may fit, or browse all order forms.
- school shirt order form
When you are selling shirts by size to one class or team.
- golf outing sign-up
For a charity golf day, where teams and sponsors sign up rather than buy goods.
- craft fair vendor application
When you run a fundraising fair and choose which vendors get a stall.
