What we left out
- A payer’s name and title. The captain or the sponsor’s contact is the person you invoice.
- A postal address for every player. Nothing is posted. The captain’s email is enough to reach the team.
- Division questions for competitive players. Age, level of play and skills scores belong to a sports competition entry, not a fundraiser.

Cap the field
Settings → Limit responses counts entries, and a pair, a single or a sponsor is an entry too, so set it above your number of teams. Close it under Settings → Schedule before you make the pairing sheet.

Questions about this template
Can single players enter without a team?
Yes. “Just me” skips the player boxes, and the thank-you screen says you will place them in a team.
Can sponsors sign up on the same form?
Yes, with or without a team. “Sponsoring only” skips the golf questions. Any package asks for the company and artwork and ends on a sponsor thank-you screen with the artwork deadline.
Can teams pay their entry fee on the form?
No. PulseFormCreator does not take payments. The fee box says how to pay, and the thank-you screen carries your payment link or bank details.
Why is the handicap optional?
Many charity scrambles do not flight by handicap, and plenty of social golfers do not have one. Make it required if you do.
