What this form asks, and why

A model release names the photographer, the model and the shoot, records any fee, lists the uses the model allows and for how long, and says how they are credited. It adds a parent or guardian for anyone under 18 and ends with the model’s signature and the date.

The last column shows how often real model release forms ask for it.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When “Are you 18 or older?” is No

shows “Parent or guardian’s name” and “Relationship to the model”

  2. When “Payment for this release” is “A fee”

    shows “Fee agreed”

  3. When “Where may the images be used?” includes “Stock licensing to third parties”

shows “Stock agencies may also ask you to sign their own release”

  4. When “How long may the images be used?” is “A set number of years”

    shows “How many years?”

Make it yours

  1. One link per shoot

Add ?shoot=Spring lookbook to the link and the shoot fills itself in. Each model signs their own response, so releases group by shoot in your results.

  2. Sign on set

Use Get QR code in the Share tab, print it and tape it by the door. Models sign on their own phones between looks.

  3. Keep it off the open web

Settings → Password protection means only people you give the password to can open the release.

Questions about this template

Do I need a model release if the model is not paid?

Payment does not decide it; the use does. Choose “No payment” and the release says so plainly instead of hiding it in “valuable consideration”. For a customer’s quote, use a testimonial release form.

Do models sign a new release for every shoot?

It is cleaner. The release names the shoot, so a new shoot means a new response, grouped by its own link.

Can a model under 18 sign?

Not alone. Answer No to “Are you 18 or older?” and the form asks for a parent or guardian, who signs.

Is this a 2257 release?

No. US producers of sexually explicit material must check and keep each performer’s ID, and this form does neither.

How does the model get a copy?

On Free, turn on the “Email me a copy of my answers” button for the thank-you screen. It sends them a link to what they signed.

Which form do you need?

Not quite the job? One of these may fit, or browse all release and consent forms.

  • Media release form

When a school, charity or event wants people’s photos for its own channels, with a way to withdraw.

  • Release of liability form

When the shoot itself carries physical risk, such as water, heights or stunts.

  • Audition form

When you are still casting and need people’s details before anyone is photographed.
