What happens after someone submits

Set it up once in the Workflows tab, and the office sees every slip as it comes in.

  1. Submitted

  2. A parent signs the slip

The office gets it by email, with a PDF of the signed slip to print for the trip folder.

  3. A child needs medication

A workflow emails the trip lead only when the medication answer is Yes, with the dose and time.

  4. Slips keep coming in

Responses go to Google Sheets, so the trip lead has a live list of who is going.

What this form asks, and why

A field trip permission slip tells parents where and when the trip is, how children travel, who is in charge and what it costs. It then gets a yes or no, health notes, a phone number for the day and a signature. 3 of 18 forms are tear-offs, like the printable here.

The last column shows how often real field trip permission slips ask for it.

What we left out

  • Medical insurance and policy number. Not needed for a day trip, and it turns the slip into a health record. PulseFormCreator is not HIPAA compliant.
  • Date of birth and home address. The school already has both.
  • A parent indemnity. It alarms parents, and in California the waiver for school trips is already in the Education Code.
  • Payment details. Put the cost and a link to your school’s payment page in the trip details paragraph.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When “Will your child need medication during the trip?” is Yes

shows “Medication name, dose and time”

  2. When “May your child go on this trip?” is No

shows the “Thanks, we’ll plan for Noah to stay at school” ending

Make it yours

  1. One place per seat

Settings → Limit responses to the seats on the bus, and the slip closes when they are gone.

  2. Close on the return-by date

Settings → Schedule stops the slip taking answers after the date you give parents.

  3. One form, every class

Add ?class= and the class name to each class’s link, and the class box fills itself in.

Good practice and the law

This is general information, not legal advice. Rules differ by country and state, so check anything important where you operate.

  • UK

In England, schools always need written consent for nursery-age children; for older pupils, for higher-risk trips and trips outside school hours. DfE, health and safety on educational visits

  • US

In California, everyone on a school field trip is deemed to have waived claims against the district, and no pupil may be kept off a trip for lack of money. California Education Code §35330

  • Australia

Victorian government schools need written or electronic consent for day excursions, overnight stays and adventure activities, and must tell parents the activities, risks, transport and supervision. Victorian Department of Education, excursions

Questions about this template

Can parents sign a permission slip online?

Yes. They draw or type their signature on screen from the link in your class message, and each response records how and when they signed.

Can a student sign their own permission slip?

At 18, usually yes, though your school’s policy decides. 4 of 18 forms have the student sign too; pick “Older students” above the preview for a student declaration.

What happens when a parent says no?

They see a screen saying their child will stay at school, and you have the answer in writing instead of a missing slip.

Does one slip cover the whole year?

Use one per trip, so parents know what they are agreeing to. English and Victorian guidance allow one consent for routine local visits, with notice before each. Year-round contacts go on an emergency contact information form.

Which form do you need?

Not quite the job? One of these may fit, or browse all release and consent forms.
