A message arrives
AI triage sums it up, sets its priority and flags it if it needs a reply.
-
You reply
Answer from the message itself, and their reply comes back into the same thread.

They write again
People shows every earlier message and reply on one timeline, so nobody explains twice.
What this form asks, and why
Only what a reply needs: who is writing, where to reply, a phone number for people who would rather talk, and the message. Topic lists, company questions and uploads belong in the versions built for them, because every extra question on a contact form costs you messages.
The last column shows how often real contact forms ask for it.
| Question | Why it is there | Real forms |
|---|---|---|
| Your name Required | 29 of 29 forms ask for one. A single box suits every naming tradition. | 29 of 29 forms |
| Email Required | Where your reply goes. The hint promises it is only used to reply, as GOV.UK’s does. | 29 of 29 forms |
| Phone (optional) | Optional on purpose: 13 of 29 forms ask for one and most leave it optional. The call-back version requires it. | 13 of 29 forms |
| Your message Required | The point of the form, on 28 of 29 forms. Capped at 1,200 characters, GOV.UK’s limit. | 28 of 29 forms |
What we left out
- A topic list. 12 of 29 forms ask what the message is about, yet almost none then ask anything different per topic. The inquiry form does that.
- A CAPTCHA. 8 of 29 forms make people solve a sum, type a code or pass a challenge, which stops people as well as bots.
- A file upload. None of the 29 forms has one. Only the support version asks for a photo, because a damaged order is easier to show than describe.
- Marketing checkboxes. 5 of 29 forms slip a newsletter box in, all British or Australian. Put a newsletter signup beside the form instead.

Answer from your inbox
Turn on Email me in the Workflows tab and each message arrives with the sender’s address in it. On Pro, set Reply-To to the Email question and pressing Reply writes straight back to them.
-
See which page they wrote from
Under Hidden fields in Settings, “Capture the page the form is embedded on” is already on, so each message says which page it came from.
-
Put it on your site
Embed it with Standard embed on your Contact page or Side tab on every page. You can see the snippet before you sign up.
-
Promise a reply time you can keep
Change “one business day” in the description and on the thank-you screen.

Questions about this template
Which version should I start from?
Standard suits most Contact pages. Pick Call-back request when people would rather talk, Support request when they have an order number, Business inquiry for suppliers and partners, and Minimal for a footer. If each topic needs its own questions, use the inquiry form.
Why is there no CAPTCHA on this form?
A hidden bot trap and a three-second minimum do that job instead, and what they catch waits in the Spam tab, not your inbox. Both are already on, under Spam protection in Settings.
Does the sender get a copy of their message?
Add the “Email me a copy of my answers” button to the thank-you screen and they are emailed a link to a PDF of their message, free. On Pro, Email the respondent attaches the PDF to a confirmation.
Can someone write without wanting a reply?
Yes, if you make Email optional. Orange Sky Australia asks “Would you like to be contacted?” so people can leave feedback without one.
Which form do you need?
Not quite the job? One of these may fit, or browse all contact and feedback forms.
- general inquiry form
When different kinds of inquiry need different questions, such as an order number or a press deadline.
- lead capture form
When the message is a sales lead and you want company, team size and timing first.
- request a quote form
When people want a price and you need the scope, site, files and deadline.
