What this form asks, and why
Build it the way browsers expect: the country first, then the parts in the order that country writes them, with address line 2 always optional. Each box tells the browser what it holds (address-line1, address-line2, address-level2 for the city, address-level1 for the state, postal-code, country), so autofill works, and US ZIP codes take five or nine digits.
The last column shows how often real address forms ask for it.
| Question | Why it is there | Real forms |
|---|---|---|
| Name(s) as they should appear on the envelope Required | One box handles “The Garcia family” and “Dr. Ana Lee & Sam Lee”, which first and last name boxes cannot. 15 of 15 forms ask for a name. | 15 of 15 forms |
| Mailing address Required | The point of the form, on 15 of 15 forms. Separate boxes for street, city, state and ZIP or postal code mean nobody forgets a line, and the browser can fill them in. | 15 of 15 forms |
| Email (optional) | Optional, for chasing a missing unit number. 9 of 15 forms ask for one. | 9 of 15 forms |
| Phone (optional) | Optional for cards and invitations. The Shipping preset makes it required, because couriers ask. | 11 of 15 forms |
| Anything we should know? (optional) | Catches a move before the envelopes go out, without the “date the new address starts” box on 4 of 15 forms. | 4 of 15 forms |
What we left out
- A signature. 12 of 15 forms have one, because they change an account or a legal record. A guest list is neither.
- ID numbers and date of birth. 3 of 15 forms ask for a government ID number and 3 of 15 forms for a birth date, just to change where mail goes. Nobody needs either to mail a card.
- The old address. 6 of 15 forms ask for it because they update a record. A new list has nothing to update.

Make it yours
-
Set your country
Pick the address question’s Default country in its settings. People abroad can still choose theirs, and the state and ZIP or postal code boxes adjust.
-
One link per side of the family
Add ?list=Work friends or ?list=Family to each link you send. The hidden list field records it, so you can sort the replies later.
-
Pick the preset
“Directory or class list” adds a former name and a sharing question; “Shipping details” makes phone required and asks for delivery instructions.
-
Close it when the envelopes go out
In Settings, turn on Schedule and set it to close on your mailing date.

Questions about this template
Where do I get a change of address form for the post office or the IRS?
From them: USPS, the IRS (Form 8822) and your motor vehicle office each have their own. This template collects other people’s addresses. Staff updating their own details use the employee information update form.
Can I print envelope labels from the responses?
Yes, from the CSV or Excel export: one row per response, the envelope name in one cell and the whole address on one line in the next (“14 Harrow Street, Apt 3, Portland OR 97214, United States”). If your label tool wants street, city and ZIP code apart, split that cell first.
Does it work for addresses outside the US?
Yes. People pick their country first and the boxes change to fit it: a county for the UK, a province for Canada, an Eircode for Ireland, and a general layout for countries without their own.
Can a family send one address for everyone?
Yes. That is what the envelope name is for: “The Nguyen family” is one response and one envelope.
Which form do you need?
Not quite the job? One of these may fit, or browse all contact and feedback forms.
- online RSVP form
When the invitation has gone out and you need who is coming, how many and meal choices.
- newsletter signup form
When you will email people rather than mail them something.
- simple contact form
When people have a message for you rather than an address to give.
