What is this about? (optional)

Your feedback (required)

One specific example helps more than a general comment.

Which area does it relate to? (optional)

Your role (optional)

Broad on purpose. Skip it if you would rather not say.

Would you like a reply? (optional)

What you get back

Every response lands in your results the moment it is sent. Turn on email notifications and each one arrives like this, and the same answers become a row in your spreadsheet.

What happens after someone submits

An anonymous form builds no contact list, and most responses can’t be answered.

  1. Submitted

  2. Feedback arrives

AI triage sums up each response and tags its theme, without seeing an email address or which link it came from.

  3. Once a day or once a week

A digest counts the new feedback and lists anything urgent.

  4. Someone asks for a reply

Answer from their response, and their reply comes back into the same thread.

What this form asks, and why

Questions about things people see and could change, one box for a specific example, and nothing only one person could answer. Ask “What should we stop doing?” or “What would make your week easier?”, never for a project name, a shift or a date that points to one person.

Question Why it is there
What is this about? (optional) Optional, and about the feedback rather than the person.
Your feedback Required The only required question. The hint asks for one example, which is easier to act on than a general complaint.
Which area does it relate to? (optional) Sends a concern to the right team without asking who wrote it.
Your role (optional) Useful for reporting, and the answer most likely to give someone away. Delete it if any option would cover only a handful of people.
Would you like a reply? (optional) Makes giving up anonymity a deliberate choice rather than a default.
Email for the reply Required Only when “Would you like a reply? (optional)” is Yes Shown only after a yes, with a note that says plainly an email identifies the sender.

What we left out

  • A required name or email. A required identity field makes the form’s title untrue. Replies are opt-in instead.
  • A file upload. Photos and documents can carry author names, device and location details, and screenshots show whose account took them.

How the form changes as people answer

Most people see a shorter form. These questions and screens only appear when an answer calls for them.

  1. When “What is this about? (optional)” is “A concern”

shows “An anonymous message cannot be followed up”

  2. When “Would you like a reply? (optional)” is Yes

    shows “Email for the reply”

  3. When “Email for the reply” is answered

shows the “Thanks, we’ll reply” ending

Make it yours

  1. Say who reads it

Name who reads the responses in the opening note and on the thank-you screen, so people know whose inbox their words land in.

  2. Name the formal channel

The note shown for concerns points to HR or the whistleblowing line. Name yours so serious matters go where they can be investigated.

  3. One link per group, never per person

Add ?group=warehouse to a link and every response from it carries that group. Never put a name in a link: it identifies whoever uses it.

  4. Switch the wording for a school or club

Pick “School, club or community” above the preview for parent, student and member roles and the safeguarding lead as the channel.

Questions about this template

Can I see who sent a response?

Not unless they leave an email for a reply. With Anonymous responses on, the form keeps the answers, when they were sent and the ?group= value from the link, but no IP address (not even as a hash), browser, device or location.

What can still give someone away?

What they write, an email they choose to leave, and small groups: in a team of four, “Manager” is a name. The send time is kept, so avoid sending the link to one person at a time.

Can I run it as an anonymous survey?

Yes. Add a rating question above the feedback box, keep Anonymous responses on and send one link to everyone. Results shows the average rating, and Compare by splits it by the group from each link. For a class, the course feedback form is anonymous already.

What should I do with the feedback?

Read it the same week, then tell the whole group what you heard, what will change and what won’t. You can’t answer anyone one by one, so that update is the only reply most people get.

Is anonymous the same as confidential?

No. Anonymous means you do not know who wrote it; confidential means you know and will not tell. This form is anonymous unless someone leaves an email.

Which form do you need?

Not quite the job? One of these may fit, or browse all contact and feedback forms.
